AVCC offers Adult Residential Services that range from minimum supports and supervision to twenty-four hour on-site supervision and direct care. A continuum of special programs is offered and designed to address individual needs and potential.

Residential Services are funded through the Colorado Division of Developmental Disabilities (DDD) Comprehensive Block funding and may be also referred to as Comprehensive Services Waiver.

These programs include the following:

  •   Group Residential Services and Support

Designed to provide 24-hour on-site supervision, services, and supports for 4 - 8 individuals living in a group home. Our services and supports include on-site, direct care for medical or physical needs and supervision and supports for emotional, social, and daily living needs. Individuals participate in AVCC day programming facilities located in La Junta, Las Animas, and Rocky Ford, Colorado.

  •   Individual Residential Services and Support

Provides personalized services and supports to individuals living in his/her own community apartment or home, an agency-owned or leased home, or a Host Home Provider setting. No more than three persons may live in any one house (PCA - Personal Care Alternative) sharing the same roof. Each individual has access to 24-hour support, when needed, as well as staff and/or providers to assist them in their home, daily and community living needs.

  •   Host Home Provider Services

Provided to individuals who desire to live in a family-like environment. The program allows for individuals from the community to accept 1 – 3 adults into their private homes and provide residential services to them. A daily reimbursement rate per individual is paid to the provider for services. The provider must meet all state and agency certification requirements. The range of care may vary from a moderate level of care to total care. Providers are responsible for meeting the physical and emotional needs of residents.
