The Arkansas Valley Community Center provides case management services to all eligible persons living in Otero, Crowley, and Western Bent Counties.  The Case Management department is often the first point of contact for individuals and families seeking services for both children and adults.  First, a case manager will meet with you to gather information and explain services available through AVCC and other community partners.  The information gathered will be used to determine a person’s eligibility for services.  If eligible, all persons are entitled to have an Individualized Plan (I.P.).  During the I.P. process, Case Managers help individuals make decisions about services and supports that will help them be successful.  Case Managers also monitor to ensure services are effective. There may be a waiting list for services so Case Managers also assist individuals and their families in connecting with other community services and supports.

 

Eligibility

Eligibility for services is based upon having a developmental disability prior to a person’s 22nd birthday.  Developmental disability means a person has a cognitive impairment attributed to mental retardation or similar conditions that causes significant skill deficits in at least two of the following areas: communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure, and work.
